Substance Abuse Case Manager

POSITION SUMMARY: Responsible for providing comprehensive case management services to patients with a history of substance abuse and mental health concerns. The case manager will provide services at both the Lemp and Biddle locations.

Education: Masters of Social Work required

Experience:

1. Two years experience in a health care or mental health field setting providing direct services including but not limited to: intake, diagnostic, screening, crisis intervention, substance abuse treatment, trauma informed and therapeutic services.
2. One to three years experience working in a community health center preferred.
3. One to three years experience working with high risk population preferred.
4. One to three years experience working with Medication Assisted Treatment (MAT) preferred.

• Works well on a team as well as independently
• Good written and oral communication skills
• Good computer skills including the use of word processing and e-mail

Residency and Transportation Requirement: Must possess a valid driver's license and automobile with insurance coverage as required by the State of Missouri.

License(s)/Certification(s) Required:  LCSW - Licensed Clinical Social Worker; CPR Certification Required

Essential Functions

• Primarily provide case management for patients in the Medication Assisted Treatment Program
• Secondarily provide case management to adult behavioral health patients
• Work closely with the Medication Assisted Treatment team to complete patient intake
• Coordinate patient treatment plan with the multidisciplinary team
• Provide crisis intervention including risk assessment to patients referred by their PCP.
• Provide substance abuse screening including MOSBIRT
• Refer patients to appropriate resource(s) when further services are required
• Familiarity with community resources
• Maintain documentation of patients in the electronic medical record
• Perform other duties as assigned.
