RN and LPN Clinic Nurse Coordinator - Immunization

POSITION SUMMARY: The primary responsibility of the Immunization Clinic Nurse Coordinator is to work with the internal departments of Affinia Healthcare, external partners, and the community to coordinate services and events to increase immunization rates of adult and pediatric populations. The Immunization Clinic Nurse Coordinator supervises the immunization team, and also administers vaccines to adult and pediatric patients, tracks and documents vaccines in the electronic medical record, and the State of Missouri immunization portal, and perform the duties required to coordinate and monitor provision of medical services in a professional manner for each individual patient.

Education: Licensed Practical Nurse, Associates degree or Bachelors of Science in Nursing.

Experience: Three (3) years as an LPN or RN in community clinic, public health nursing, and/or hospital setting;

Skills and Abilities (which may be representative but not all inclusive of those commonly, associated with this position):

1. Strong verbal and written communication skills required.
2. Ability to work effectively individually as well as a team member with physicians and other staff.
3. Ability to appropriately interact and educate patients, families, and staff.
4. Experience with Electronic Medical Record (EMR) and Microsoft office applications such as Outlook, Word and PowerPoint.
5. Experience administering immunizations to pediatric and adult populations
6. Skills required for reagent handling, labeling, processing, preparations, transportation and storage necessary
7. Must be knowledgeable of required regulations and comply with them.
8. Ability to coordinate the activities of a busy provider practice.
9. Ability to maintain confidentiality

Transportation Requirement: Must possess a valid driver's license and automobile with insurance coverage as required by the State of Missouri.

License(s)/Certificate(s): Licensed as LPN or RN in the State of Missouri.

Essential Functions

1. Provide oversight of immunization clinics
2. Train new members of the team
3. Supervise medical immunization team
4. Report immunizations to the ShowMeVax
5. Responsible for the execution of the City of St. Louis Department of Health Immunization grant
6. Assists with implementing new immunization clinic sites
7. Prepares immunization reports for the city DOH
8. Coordinates services with the city DOH
9. Daily management of immunizations, documenting, and reporting
10. Assists with ordering and administration of COVID vaccine
11. Coordinate community immunization events
12. Communicate and coordinates with partners for community immunization needs and events
13. Closes care gaps through scheduling patients for appointments with provider
14. Administer immunization shots as dictated by state regulations
15. Coordinate and monitor provisions of medical services.
16. Assist providers, take and record vital signs, abnormal results, distribute sample medications and administer injections per provider's orders.
17. Triage walk-in clients, call-in clients as appropriate for patient care and safety.
18. Keep patients records; complete appropriate forms and reports. Provide appropriate documentation. Adhere to patient confidentiality.
19. Complies with all designated safety policy and procedures in the work area including the use of applicable personal protective equipment when necessary to prevent exposure to potentially infectious agents.
20. Provide health education and counsel patients in provider instructions and treatment program. Coordinate nursing care plans with other team members.
21. Assist with implementation, monitoring, and evaluation of Quality Improvement Programs.
22. Observe sign and symptoms, document and report reactions of the immunizations to the appropriate provider.
23. Perform other duties as assigned.
