Population Health Analyst
POSITION SUMMARY: The Population Health Analyst is an autonomous position responsible for community and clinical expertise on specific, complex, and rising risk patient populations with a design to meet specific contractual and program related requirements. This role will perform data collection, validation, and narration often as part of interdisciplinary teams. This role ensures excellence in recognizing patient population needs, identifies community connections to mitigate barriers, and guides optimal patient-centered clinical outcomes.
POSITION QUALIFICATION REQUIREMENTS
Education: Bachelor's degree required. Knowledge of theories, principles, and methods utilized in the development of health metrics typically acquired through completion of key certifications or advanced degree.
Experience:
- Two years of related experience in a health care-related setting or a related advanced health industry degree is preferred.
- Experience with data interpretation, tools (run, control, and Pareto charts), and data extraction preferred.
- Excel; Access; SAS SQL; SPSS; MS Office; knowledge.
Skills and Abilities which may be representative but not all inclusive of those commonly associated with this position:
- Good oral and written communication skills
- Thorough understanding of population health concepts
- Comfortable leading and participating on multidisciplinary teams.
- Ability to exercise judgment and prepare supporting or correcting documentation as deemed appropriate.
- Initiative-taking to explore documentation detail through research and internal networking.
- Possess a community minded approach to problem solving.
Residency and Transportation Requirement: Must possess a valid driver's license and automobile with insurance coverage as required by the State of Missouri.
License(s)/Certification(s) Required:
- Healthcare Analytics Certification or CHDA preferred.
- Six Sigma Green Belt or Lean Healthcare Certification preferred.
- NextGen E.H.R. Certified preferred.
FUNCTIONS OF THE JOB
Essential Functions
- Actively manage population health initiatives using project management, lean methodologies, performance data, and qualitative or quantitative metrics as appropriate.
- Engage equitable approaches to address health barriers and identified concerns effectively and efficiently.
- Interface with key providers (e.g., dental hygienists, behavioral health, physicians, community health team, etc.) as well as other community resources to assure that patients are linked to and engaged in services can be tracked.
- Provide guidance, analysis, management, and methodological support for the development of measurement plans for new performance indicators.
- Ensure defined safety and quality standards are met within area of responsibility.
- Evaluate and manage customer satisfaction data to improve systems, processes, and outcomes toward patient expectations and needs.
- Develop goals consistent with institutional and departmental goals and policy.
- Provide interpretation and presentations to others (oral or written) to tell the population health story.
