Patient Support Services and Managed Care Coordinator

Supervises: Managed Care Patient Navigator, Managed Care Patient Coordinator, Patient Support Representative, Patient Support Coordinator

POSITION SUMMARY: Responsible for oversight, coordination and monitoring of scheduling activities as performed by the Patient Support Service Representatives and Managed Care Patient Navigators. Routinely measures and analyzes quality indicators for quality improvement and schedule adherence. Performs auditing, reporting and required training to ensure efficient processing of scheduling among team.

POSITION QUALIFICATION REQUIREMENTS

Education:
1. Minimum high school graduate or equivalent, Associates or Bachelor's degree
2. Graduate of a certified Medical Assistant program required

Experience: Minimum three (3) years' experience in a clinical setting required. Minimum three (3) years EMR and patient scheduling/patient support experience required.

Transportation Requirement: Must possess a valid driver's license and automobile with insurance coverage as required by the State of Missouri.

FUNCTIONS OF THE JOB
Essential Functions
1. Monitor the centralized scheduling phone software including staff assignments, cross coverage and cross training.
2. Monitor a caseload of managed care patients for compliance with appointments, prescriptions, immunizations, education and health/dental care utilization.
3. Communicate noncompliance to members of the health care and dental team for appropriate intervention
4. Schedule patient appointments, make reminder appointment calls, contact patients that have missed scheduled appointments
5. Assigns managed scheduling offline assignments and provides coverage for scheduled and unscheduled patient support representative absences and/or managed care navigator absences.
6. Serve as the organization and team trainer for appointment scheduling and portal usage.
7. Responsible for improving communication and collaboration among the patient support representatives, managed care navigators and other departments.
8. Provides input and assistance of annual evaluations for patient support representatives and managed care patient navigators as directed.
9. Responsible for identifying and resolving any barriers to timely scheduling and gap closures and reporting any concerns to Chief Medical Officer and others as requested for timely interventions.
10. Act as front-line support for escalated calls and customer service resolutions.
11. Provides quality monitoring of calls and appointments scheduling within patient support services department.
12. Communicate with patients to determine barriers to care (transportation, access, etc.)
13. Monitors all care gap reports among managed care plans to include timeliness in the completion rate and deadline.
14. Troubleshoots schedules with EMR and corrects and publishes as needed.
15. Work independently within a team environment
16. Perform other duties as assigned

Marginal Functions:
1. Attend meetings as required.
2. Assist in collecting data for the Chief Medical Officer as it relates to Provider's schedules.
3. Assist providers in completing all medical documentation needed to close care gaps.
4. Maintain training materials for Patient Support Representatives and Managed Care Navigators.
