Maternal Child Nurse Case Manager

POSITION SUMMARY: Perform the duties required to oversee the provision of case management services to pregnant women and various categories of children receiving primary health care at Affinia Healthcare.

POSITION QUALIFICATION REQUIREMENTS

Education: Requires LPN license or Bachelor of Science Degree in Nursing

Experience:

  1. Three (3) years' experience in primary care, maternal child, or community health nursing.
  2. One (1) years' experience in supervision and/or nursing education.
  3. On (1) years' experience in case management (preferred).

Skills and Abilities which may be representative but not all inclusive of those commonly associated with this position:

  1. Strong computer skills including the use of word processing, spreadsheet, database, and e-mail software.
  2. Excellent oral and written communication skills including the preparation of proposals.
  3. Able to work with people with diverse ethnic backgrounds.
  4. Good analytical and problem solving skills.
  5. Able to work on multiple tasks simultaneously.

Residency and Transportation Requirement: Must possess a valid driver's license and automobile with insurance coverage as required by the State of Missouri. .

License(s)/Certification(s) Required:

  1. LPN or Registered nurse licensed to practice in the State of Missouri
  2. CPR certified

FUNCTIONS OF THE JOB

Essential Functions

  1. Assesses patients' physical and psychosocial status and obtains data regarding development status and behavior to include the care of the newborn, pediatric, anti-partum, and post-partum patient. .
  2. Plans and implements patient care in collaboration with the physician.
  3. Evaluates and updates appropriateness of care plan and physician orders.
  1. Communicates appropriate information to patients, peers, managers, physicians, department, and other agencies involved.
  2. Integrates problem solving methodology and quality improvement in approaching problem areas related to patient care and organizational processes.
  3. Completes documentation in clinical records, time sheets and quality control logs.
  4. Ensures confidentiality of employee, legal, client/patient, budget and all company matters.
  5. Provides nursing care while promoting client/patient dignity and provides a calm, supportive and therapeutic environment for client/patients and their families.
  6. Acts as a resource to others (nursing students, peers, and other departments) in provision of client/patient care.
  7. Perform other duties as assigned.

Marginal Functions

  1. Attend staff and agency meeting as needed
  2. Serve on committees as requested.
  3. Represent Affinia Healthcare in the community and make presentations as requested.
  4. Monitor departmental purchases and expenditures.
