MAT Nurse Coordinator

POSITION SUMMARY: The MAT Nurse Coordinator will perform the duties required to coordinate and monitor the provision of Medication Assisted Therapy (MAT) for patients receiving outpatient treatment for substance abuse. The MAT Nurse Coordinator will also perform the duties of a Clinic Nurse Coordinator in providing medical care and coordination services for all patients.

Education:  Bachelor of Science Degree in Nursing required.

Experience:

• 3 years experience in primary care, behavioral health, community health, or related field.
• 1 years experience in substance abuse or addiction environment (preferred).
• 3 years experience in case management (preferred).

7. Knowledge of crisis intervention
8. Ability to work independently, as well as with a team.

Residency and Transportation Requirement: Must possess a valid driver's license and automobile with insurance coverage as required by the State of Missouri.

License(s)/Certification(s) Required:  Registered nurse licensed to practice in the State of Missouri; CPR Certification

Essential Functions:

• Performs initial medical triage on patients requesting medication assisted therapy services.
• Assesses patients 'physical/substance use disorder status, analyzes data, and integrates assessment resulting in planning for the care of the outpatient medication assisted therapy patient
• Demonstrates critical thinking skills to achieve outcomes and solve problems.
• Coordinates the management and administration of medications prescribed by the MAT provider including inventory, ordering, compliance, and vendor agreements.
• Performs medication authorization and refills.
• Performs urine drug screening and monitoring, and provides all reports to the MAT provider and team members.
• Observe sign and symptoms of medical issues, documents, and reports reactions of the treatment to the provider.
• Delivers organized care by prioritizing care needs and tasks effectively. Adapts care to changes in patient's condition.
• Communicates with patients, team members, in an accurate, timely, and professional manner.
• Provides health education and counsel patients and family members in medication treatment, substance abuse.
• Collaborates with multidisciplinary team and coordinates further substance abuse services/mental health treatment as needed.
• Utilizes evidenced based practices to solve problems.
• Maintains the integrity of patient medical records, completes appropriate forms and reports accurately, and adheres to patient confidentiality.
• Performs other duties as assigned.

 Marginal Functions:

• Participate in evaluating and planning for existing and new services.
• Assist in the orientation or ongoing in-service education of physicians, LPNs, students, volunteers, and other staff
• Serves as liaison between various Affinia Healthcare initiatives, the MAT team and the community.
• Attends group team meetings other agency meetings as required.
• Perform internal and external audits of records to maintain quality and compliance standards.
