LPN or RN Chronic Disease Nurse

POSITION SUMMARY: Perform the duties required to coordinate develop, manage, implement, and monitor services for Chronic Disease Department and Chronic Disease Management programs. Provide direct supervision of the Health Coaches and BCCCP Follow-Up Assistant.

POSITION QUALIFICATION REQUIREMENTS

Education: Licensed Practical Nurse (LPN), Associates or Bachelors of Science Degree in Nursing

Experience:

1. Three (3) years' experience in ambulatory care, hospital and/or community health nursing.
2. One (l) year experience in program planning/development, and management experience in ambulatory care, hospital or community health nursing.

3. Able to work with people with diverse ethnic backgrounds.
4. Strong computer skills including the use of word process, spreadsheet and Email software.

Residency and Transportation Requirement: Must possess a valid driver's license and automobile with insurance coverage as required by the State of Missouri.

License(s)/Certification(s) Required:

1. Licensed Practical Nurse or RN in Missouri
2. CPR certified

FUNCTIONS OF THE JOB

Essential Functions

1. Manage the Chronic Disease Management initiatives for Affinia Healthcare to include recruiting, and evaluating staff.
2. Coordinate development of chronic disease program protocols and standards of care.
3. Monitor and distribute chronic disease program patient referrals to staff.
4. Assist in developing new programs using the proven process improvement methodologies.
5. Consult on various chronic disease projects, i.e. Women's Health, Smoking Cessation, Cardiovascular Disease, etc., and serve as team leader and/or member on designated projects.
6. Plan and coordinate staff in-services on programming.
7. Establish program goals and objectives for chronic disease management programs, and formulate reports.
8. Manage the development, implementation, monitoring and evaluation of educational curricula for Health Coaches.
9. Work in coordination with health center staff to monitor and maintain adherence of programming to granting bodies standards.
10. Complete monthly reports (travel summaries, encounters, program updates).
11. Monitor health coaches' compliance to established policy and procedures.
12. Assist in the development and implementation of policies and procedures for Chronic Disease department and other areas as required.
13. Coordinate and oversee special programs as assigned.
14. Prepare and maintain needed records and reports.
15. Prepare and revise educational materials for clients with chronic diseases.
16. Oversee the follow up activities provided through BCCCP.
17. Participate as a member of the Chronic Disease Collaborative teams dealing with chronic diseases.
18. Work in coordination with health center nurse coordinators and supervisors to monitor and maintain adherence to NCQA, Joint Commission, and other accrediting body standards.
19. Represent Affinia Healthcare in the community and make presentations as requested.
20. Monitor the goal achievement of chronic disease programming and other grants and contracts involving coaching of clients with chronic diseases.
21. Participate in the planning and evaluation of new and existing programs.
22. Perform other duties as assigned.

Marginal Functions: Orientates volunteers, physicians, nurse instructors and students.
