Health Outreach Worker - Bilingual

POSITION SUMMARY: Perform the duties required to assist students, parents and other community residents with Affinia Healthcare appointments. Assist uninsured students, parent, community residents and school personnel with applications for pertinent health care plans/programs, such as Medicaid/Mo HealthNet, CHIPs, Health Insurance Marketplace, Gateway To Better Health Program, and other health care benefits programs available to Affinia Healthcare patients. Attend designated school district events to promote Affinia Healthcare school-based services.

Hours: Full-time (Evenings and Weekends as scheduled)

Education: Requires high school diploma or equivalent.

Experience: Requires one (1) year of community health, community outreach or related field.

Skills and Abilities:

1. Fluent in Spanish and English required
2. Strong verbal and written communication skills.
3. Good organizational skills and interpersonal skills.
4. Good computer skills including the use of word processing and e-mail software.

Transportation Requirement: Must possess a valid driver's license and automobile with insurance coverage as required by the State of Missouri.

License(s)/Certification(s) Required: CPR Certification required, Health Insurance Marketplace Certification

Essential Functions

1. Contacts and recruits to promote the Health Center services.
2. Submit patient appointment requests to the appointment staff. Track and monitor kept appointments.
3. Recruits and assists with enrollment in the MO Health Net Programs, Presumptive Eligibility, DOSA, Temporary Medicaid and utilizes the EMOMED MO Health Net provider system - for approval status.
4. Recruits for participation in Community fairs, Outreach Medicaid, special Dental activities, and Pediatrics
5. Distributes health center materials (fliers) in neighborhood sites, such as stores, homes, and schools.
6. Enters data into the NEXTGEN system.
7. Keeps daily logs of phone calls, home visits, other contacts, and monthly statistical reports.
8. Conducts surveys and assists with monitoring patient/client flow in waiting area as required.
9. Performs other duties as assigned.

12. Attends Health Center and community events as assigned.
13. Attends training sessions and all meetings as required
