Early Intervention Coordinator, HIV Grant Coordinator
POSITION SUMMARY: The Early Intervention Coordinator, HIV Grant Coordinator ("EiC") will support the daily operations of Affinia Community Health (ACH) HIV program. The EiC is a member of the Community Health Department, working in close collaboration with providers, clinical and non- clinical staff, to ensure adherence to program standards. The EiC will act as a liaison between Affinia Community Health departments and external organizations requiring or requesting information regarding Affinia Community Health programs. This position is responsible for: Planning, coordinating, and overseeing HIV Program while setting priorities for managing the program. Monitoring, maintaining, and scheduling Affinia Community Health-HIV testing services to different locations within Affinia Healthcare and different community partners. Oversees daily operations and provide health literacy education to clients. Conducts outreach and social marketing in the community. Maintain confidentially at all times when discussing and maintaining up-to-date client documentation. Analyze community data to identify gaps in service. The EiC will also assist with facilitation of new site implementation and site approval processes, ordering supplies for various locations, on-boarding/training of new Early-Intervention-CHW and CHW staff, in addition to other duties assigned by the Asst. VP, Clinical Integration.
POSITION QUALIFICATION REQUIREMENTS
Education:
High School diploma
Associates in Healthcare or IT, or Bachelor's degree preferred
Experience:
Three (3) years' experience in community health setting required
Three (3) years' experience in a clinical setting required.
Previous coordinator experience preferred.

FUNCTIONS OF THE JOB:
- Support the daily operations of Affinia Community Health programs- HIV grant and others as needed.
- Collaborate with providers, clinical teams, school based teams, homeless teams, and other pertinent non clinical staff to develop, implement, and evaluate program standards.
- Monitor and maintain Community Health databases.
- Monitor and maintain data exchange between new and existing school based health centers and corresponding school districts with Affinia Healthcare's electronic medical record.
- Monitor and update benchmark data.
- Assist with facilitation of new site implementation and site approval processes.
- Order supplies for Community Health Department.
- Participate in on-boarding/training of new school based health center and community health staff.
- Coordination for referrals to needed medical treatments or specialty care and follow up as needed.
- Provision of assistance, advice, and/or referral to resources for supportive service needs.
- Provision of treatment adherence counseling.
- Provision of variety of interventions needed to retain the patient in care.
- Provision of health education to patient to expand the patient's health literacy and improve general health.
- Other duties as assigned.
Marginal Functions:
- Attend and actively participate at required meetings.
- Assists in preparation of monthly reports regarding case management/community health worker activities.
- Assist with the implementation of new procedures and processes.
Transportation Requirement:
- Must possess a valid CDL Class B driver's license and automobile with insurance coverage as required by the State of Missouri. (Required)
