Community Health Worker/Hospital Liaison

POSITION SUMMARY: Serves the patient and medical teams by providing excellent patient care and establishing close points of contact between medical and social teams. Create clear lines of communication, closing care gaps, and decreasing social determinants of health through the circulation of important information. Perform the duties required to act as a liaison between primary care medical providers, social service agencies, community health departments, hospital systems, and local community organizations.

POSITION QUALIFICATION REQUIREMENTS:

Education: Requires high school graduate or equivalent. Medical Assistant or LPN preferred.

Experience: Requires three (3) years of experience in health care, social services, or related field.

Skills and Abilities (which may be representative but not inclusive of those commonly associated with this position).

1. Knowledge of Medicaid and MC+ programs.
2. Knowledge of health center protocols, policies, and procedures.
3. Highly proficient computer skills with Outlook, Word, Excel, and experience with an EMR.
4. Willingness to promote health and maintenance programs.
5. Demonstrates initiative, flexibility, and excellent organizational skill.
6. Strong oral and written communication skills which demonstrate excellent customer service and teamwork.
7. Ability to create a safe and comfortable environment for patients and staff.
8. Ability to maintain a professional attitude and demeanor at all times.
9. Ability to keep information confidential.

Transportation Requirement: Must possess a valid driver's license and automobile with insurance coverage as required by the State of Missouri.

License(s)/Certification(s) Required: CPR Certification

FUNCTIONS OF THE JOB

Essential Functions:

1. Acts as liaison between medical providers, insurance companies, various agencies, and hospital and primary care nurses.
2. Maintains and updates insurance information on Case Management spreadsheets; maintains information from clinic and hospital databases.
3. Participates in Prenatal Intake at the various Health Centers by coordinating the care for patients to ensuring they receive services from Outreach, WIC, Maternal Child Nursing, Centering, Dental, and Radiology.
4. Assists with the scheduling of specific patient appointments, makes reminder appointment calls, and contacts patients who have missed scheduled appointments when necessary.
5. Visit postpartum patients at the hospital to obtain necessary postpartum, delivery, and newborn information.
6. Obtains medical documents for transitions of care.
7. Responds to General Practitioner queries in a professional and timely manner.
8. Effectively works with other health care team members to ensure coordination of care.
9. Addresses the questions and concerns of patients, families, and staff members
10. Compiles and coordinates patient feedback and information for the primary care team.
11. Assists with enhancing communication between hospitals and primary care providers.
12. Performs other duties as assigned

Marginal Functions

1. Attend and actively participate at required meetings.
2. Assists in preparation of monthly reports regarding case management/community health worker activities.
3. Assist with the implementation of new procedures and processes.
