Community Health Nurse - Homeless

POSITION SUMMARY:  Perform the duties required to provide Public Health nursing, advocacy, triage and case management services to the insured, uninsured, and homeless clients.  The nurse's duties, functions, and responsibilities are in accordance with the Missouri State Board of Nursing Practice Act.

Education:   Associate's Degree in Nursing required.  Bachelor's of Science in Nursing preferred.

Experience: Two (2) years experience as a Registered Nurse in primary care, Community Health/Homeless or case management setting. 

Skills and Abilities

• Possess initiative, flexibility, and self-motivation.
• Good oral and written communication skills for patient education and transitioning patients to clinical care
• Good computer skills including the use of an electronic medical record, word processing and e-mail software.

Transportation Requirement:  Must possess a valid driver's license and automobile with insurance coverage as required by the State of Missouri.

License(s)/Certification(s) required:  Licensed as Registered Professional Nurse in the State of Missouri and CPR certified.

Essential Functions

• Develop, implement and follow-up on nursing care plans to provide nursing services in the primary care and community setting.
• Provide case management services, health education and counseling to eligible clients and the homeless populations.
• Refer clients to resources, track their use of these resources, coordinate medical/dental/social service/housing visits with transportation services and complete necessary follow-up.
• Coordinate with Health Center Directors to arrange timely clinic services for clients.
• Identify and report abnormal behavior, and signs and symptoms of illness to health care provider.
• Complete health-related forms and document nursing activities accurately and in a timely manner.
• Assist in implementing, maintaining and updating patient tracking system.
• Act as a consultant on health issues for clients and their families and other departments as indicated.
• Perform other duties as assigned.
