POSITION SUMMARY: The Community Health, Operations Assistant will support the daily operations of Affinia Community Health (ACH) programs. The Assistant is a member of the Community Health Department, working in close collaboration with clinical and non-clinical staff, to ensure adherence to program standards. The Assistant will act as a liaison between Affinia Community Health departments and external organizations requiring or requesting information regarding Affinia Community Health programs. This position shares responsibility for monitoring and maintaining Affinia Community Health databases and other programming, which reflect participant outcomes, benchmark data, and other indices as specified through MOU agreements. The Assistant will also assist with facilitation of new site implementation and site approval processes, ordering supplies for various locations, on-boarding/training of new Community Health staff, perform as a back-up driver for mobile health programs, in addition to other duties assigned by the Asst VP, Clinical Integration.

POSITION QUALIFICATION REQUIREMENTS

Education: Associates in Healthcare or experience equivalent.

Experience: Previous administrative experience required.

  8. Demonstrates computer skills including proficiency in Microsoft Excel and Access, Word, and

FUNCTIONS OF THE JOB:

  1. Support the daily operations of Affinia Community Health programs.
  2. Collaborate with clinical teams, school-based teams, homeless teams, and other pertinent non-clinical staff to develop, implement, and evaluate program standards.
  3. Assist with monitoring and maintaining Community Health databases.
  4. Monitor and maintain data exchange between new and existing school-based health centers and corresponding school districts with Affinia Healthcare's electronic medical record.
  5. Monitor and update benchmark data.
  6. Assist with facilitation of new site implementation and site approval processes.
  7. Order supplies for school-based program, homeless program, and other departments as instructed.
  8. Assist with on-boarding/training of new community health staff.
  9. Assist as back-up driver for mobile health program.
  10. Provides administrative support such as answering phones, assisting visitors, resolving administrative problems, scheduling meetings and appointments, managing calendars, expenses and travel arrangements.
  11. Writes and edits correspondence, policies, contracts, and special reports. Prepares graphs, charts, displays, and spreadsheets. Creates and maintains proposal and presentation templates.
  12. Maintains department policy and procedure manuals.
  13. Coordinate calendars and activities
  14. Access and input information in the EHB
  15. Assist with preparing proposals and grants.
  16. Performs special projects including creating presentations, planning and coordinating multiple projects, and working with various resources to gather and disseminate important information.
  17. Schedule and organize complex activities such as coordinating distribution of various data and materials, sets up meetings, and assists with agreements for travel, conferences, etc.
  18. Open, sort, and distribute incoming correspondence, including faxes and email. Respond to regularly occurring requests for information.
  19. Attend and records meeting minutes for departmental meetings and the other meetings as requested
  20. Track and monitor assignments and follow through to completion

Marginal Functions:

  1. Attend and actively participate at required meetings.
  2. Assists in preparation of monthly reports regarding case management/community health worker activities.
  3. Assist with the implementation of new procedures and processes.

Transportation Requirement:


License(s)/Certification(s) Required: Must possess a valid CDL Clas B driver's license and automobile with insurance coverage as required by the State of Missouri. Required with 90 days)
